How does it know what the watermark is?
It does not, and it does not pretend to. A watermark is an ordinary page object, so the tool looks for the behaviour instead: the same object in the same place on most pages, an object drawn faintly, or one set at an angle. Each candidate is listed with the reason it is there. Faint and angled ones start selected because that evidence is about the object itself; anything that only repeats is left unselected, because a header or a page number repeats just as faithfully.
Can it remove a watermark from a scanned document?
No, and nothing else can either without inventing what was underneath. In a scan the mark is made of the same pixels as the text, so there is nothing separate to delete. The tool tells you which pages are scans instead of quietly doing nothing.
Is the watermark really gone, or just hidden?
Gone from the file. The object is deleted from the document and then the saved bytes are opened again to confirm two things: that the mark is not there any more, and that every character and object you did not choose is still exactly where it was. If a page fails that check, its content is left untouched and the result says so.
Am I allowed to remove a watermark?
That depends on the document and on you, not on us. Removing a draft stamp from your own file or a sample mark from something you have bought is ordinary work; stripping a copyright notice from someone else's document is not. This tool does not ask, so please use it on documents you own or have the right to change.
